Topics presented and debated included:

•    Reflections on the coherence of NCD policies in Moldova
•    Introducing  WHO’s Package of Essential NCD Interventions (PEN Protocols) into everyday primary care practice
•    Patient-centred Integrated care models - where also other SDC projects focusing on mental health and cervical cancer made a contribution
•    Evidence-based, intersectoral planning and decision-making for the promotion of community health
•    Community mobilization for NCD risk reduction – involving also patients themselves so that their voices were also included.

In the context of Moldova, vulnerable people are those unable to work due to ill-health, lack of work or old age, residing in rural areas, and suffering from a chronic condition. The presenters drew the audience’s attention to continuous quality improvement in the primary health care and collaboration with the social sector can help ensure equitable and coordinated access to care and the avoidance of acute episodes. As well, the presenters showed the changing landscape of actors as they relate to health promotion for behavior change, and early steps to strengthen individual empowerment and community engagement by building health and nutritional literacy and encouraging self-management of NCDs by patients and their families. Evidence based health planning, using health profiles, promoting Health in All Policies and intersectoral collaboration were also topics of discussion.
